Home Health

Care Manager, Registered Nurse (RN)

The Registered Nurse Care Manager plans, organizes and directs home care services and is experienced in nursing. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and their families within their homes and communities.

Position Qualifications:

  1. Graduate of an accredited school of nursing.  One to two years of recent acute care experience in an institutional setting.
  2. Current licensure in State, CPR certification and valid driver’s license.
  3. Bachelor’s degree, with one year of home health care experience.
  4. Management experience not required.  Responsible for supervising home health aides.
  5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  6. Prolonged or considerable walking or standing.  Able to lift, position or transfer patients.  Able to lift supplies and equipment.  Considerable reaching, stooping, bending, kneeling or crouching.  Visual acuity and hearing to perform required nursing skills.
Physical Therapist (PT)

Job Description:

The Physical Therapist contracted or employed through Optima Home Health is responsible for providing physical therapy services and for adherence to all conditions outlined in the care plan.

Position Qualifications

  1. Possesses a degree from a Bachelor’s, Master’s or Doctorate program (depending on California Department of Public Health Licensure requirements for your graduation and certification date) as approved by accrediting agency.
  2. Licensed to practice as a physical therapist within the State.
  3. Community/home health experience is preferred.
  4. Demonstrates good verbal, written communication and organization skills.
  5. Possesses and maintains current CPR certification.
  6. Must have individual professional liability insurance as well as automobile insurance as required by California law.
  7. Must have current TB results and annual physical exam results.
Occupational Therapist (OT)

The Occupational Therapist whether contracted or employed through Parentis Health is responsible for the implementation of standards of care for occupational therapy services.

Position Qualifications:

  1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
  2. Registered with the National Registration Examination of the American Occupational Therapy Association.
  3. Demonstrates good verbal, written communication, and organizational skills.
  4. Must possess current CPR
  5. Must have individual professional liability insurance as well as automobile insurance as required by California law.
  6. Must have current TB results and annual physical exam results.
Certified Home Health Aide

Title: Certified Home Health Aide

The Certified Home Health Aide provides services to help patients with daily living.  They are under the supervision of our RN or PT to ensure the patients are getting targeted assistance with the tasks that help them where they need it.

Position Qualifications:

  1. Graduate of an accredited Home Health Aide Certification program.
  2. Current California State license as Certified Home Health Aide
  3. Current CPR certification (in-person course)
  4. Valid Driver’s License
  5. Auto Insurance
  6. Current Professional Liability Insurance
  7. Position requires considerable walking, standing, lifting (including transferring patients from bed to chair, bathing or other seated or standing positions)
Speech Therapist

The Speech Therapist contracted or employed through the organization assess and treat persons with speech, language, voice, and fluency disorders. May select alternative communication systems and teach their use. May perform research related to speech and language problems.

Position Qualifications

  1. Possesses a degree which includes an internship from an approved Master’s program in Speech-Language Pathology, or as required by applicable law or regulation.
  2. Meets the education and experience requirements for a Certificate of Clinical Competence in Speech-Language Pathology, granted by the American Speech-Language-Hearing Association (ASHA).
  3. Is certified to practice Speech-Language Pathology.
  4. A minimum of one year’s clinical experience in Speech-Language Pathology.
  5. Demonstrates good verbal and written communication, and organization skills.
  6. Possesses and maintains current CPR certification.
Licensed Clinical Social Worker (LCSW)
  1. Must have Masters Degree in Social Work (MSW)
  2. Must be licensed in California (LCSW qualification)
  3. A minimum of one year’s clinical experience in Social Work
  4. A minimum of one year’s home health experience desired
  5. Demonstrates excellent verbal and written communication and organizational skills
  6. Must possess current CPR
  7. Must have individual professional liability insurance as well as automobile insurance as required by California law.
  8. Must have current TB results and annual physical exam results.
